What you can take with you
Medical, dental, vision, EAP, HSA, FSA, disability, and supplemental benefitss end on your separation or termination date; however, you have the option to sign up for COBRA.
COBRA & Billing Services automatically mails you a COBRA election kit. COBRA lets you keep your current medical, dental, vision, and EAP coverage for a limited time by paying the full premium yourself, plus an administrative fee — it’s there to bridge the gap while you line up new coverage.
Questions? Visit cobraandbillingservices.com or call 833-874-1600.Your 401(k) is still yours — It stays invested exactly as you left it unless you actively request a distribution or roll it over to a new employer’s plan or an IRA.
Your HSA goes with you — Unlike an FSA, your Health Savings Account is entirely yours — the funds (including anything LSC contributed) stay in your HealthEquity account and remain usable tax-free for qualified expenses, no matter where you work next or even if you retire.
Life & AD&D Insurance — Your Basic and Optional Life and AD&D coverage also ends on your last day. You have 30 days to port or convert it to an individual policy through MetLife — do this promptly if you want to keep any coverage in place, since it won’t continue automatically.
Supplemental Coverages — Accident Insurance, Critical Illness, and Hospital Indemnity coverage ends on your separation date. You have the option to continue your current coverage. MetLife will send you a continuation-of-coverage letter with directions on how to continue coverage.

What ends on your last day
Flexible Spending Accounts — Your acccess to unused FSA funds ends on your separation date. You can still request reimbursement for eligible expenses incurred before your separation date — you just need to submit those claims by March 31 of the following year. Expenses incurred after your last day aren’t eligible for reimbursement.
Disability — Your Disability coverage ends on your separation date. If you are disabled at the time of your separation, your disability benefits may continue while you remain disabled. See provisions of the Disability Program for details.
