When it comes to cancer, our health care needs change. The Company offers benefits to detect and screen for cancer early through Galleri and, if diagnosed, provide personalized guidance and support to navigate treatment options through Carrum Health.

Galleri cancer detection

After age 50, the risk of cancer is 13 times greater, and when cancers are detected before they spread, the overall survival rate is 4 times higher.

With one simple blood draw, the Galleri test detects a shared cancer signal across 50+ different types of cancer. Galleri is offered as a preventive care benefit for you and your eligible dependents ages 50 or older, if enrolled in an LSC Medical Plan.

How it works

Request the test — Go to galleri.com/lsc to check your eligibility and request the Galleri test. It will take about 10 minutes to complete a health questionnaire.
- Galleri is available by prescription only. By requesting the test and completing the questionnaire, you’re requesting a prescription from an independent telemedicine provider.
Receive a collection kit — If the telemedicine provider confirms your eligibility and prescribes Galleri, you will receive a collection kit in the mail.
Provide your blood sample — A trained technician will draw your blood sample and ship the kit to the GRAIL lab.
Receive your results — You’ll get your results about 2 weeks after your blood is received at the GRAIL lab.

* The most you pay before the plan pays 100% of eligible expenses. Medical and Prescription Drug expenses are combined.

HSA Plans

* For HSA Core, no one in your family pays more than the individual deductible/OOP maximum before the plan pays 100% of their covered expenses. For HSA Value, there’s no individual cap on the deductible, but no one pays more than the individual OOP maximum.
** What you pay after the deductible is met.
*** The most you pay before the plan pays 100% of eligible expenses. Medical and Prescription Drug expenses are combined.
† If you use the ER for a non-emergency, you will be charged an additional $500 that will NOT count toward your deductible or OOP maximum.

Life & AD&D Insurance Coverage Summary

* The amount of coverage for active employees aged 65 and older is subject to annual age reductions in accordance with the plan. Please see the Life Insurance Certificate of Coverage & Schedule of Benefits on the Important Documents page for more information.
** The amount you elect for Optional Spouse Life cannot exceed the total of your employee Basic Life amount plus any Optional Employee Life coverage.

* For every $1 you contribute to your HSA, the Company matches $1 up to the coverage level match maximum each quarter for HSA Core or HSA Value medical plan participants.

If you are aged 65 or older and Medicare-eligible, you’ll need to opt out of the HSA Company contribution due to IRS rules.
