Leaves of Absence (LOA)

We recognize that extended time away from work is important and sometimes necessary due to illness, injury, or other personal circumstances that may require a leave of absence (LOA). Whether you take extended time off for military service, to stay at home and bond with a newborn, or to care for yourself or a loved one, we provide eligible employees with various LOA options.
Types of leave
- Family and Medical Leave (FMLA) — If you take an approved leave under FMLA or applicable state or local law, your coverage may continue for up to 12 weeks (or 26 weeks for qualifying military caregiver leave), or longer if required by law.
- Military LOA — Active military service with a written, granted leave. Coverage may continue for up to 24 months.
- General LOA — Documented leave of absence (other than FMLA or Military Leave).
Depending on the laws in your state of residence, you may be eligible for state-provided leave. Contact the appropriate state agency for eligibility requirements and additional information.
How to request a LOA
- The Hartford manages all leave requests:
- Submit your leave request by calling 888-437-8671 or apply online by creating an account at abilityadvantage.thehartford.com
- Complete all required steps
- The Hartford evaluates your eligibility and notifies you once a decision is made
- You may also qualify for state-provided leave — contact your state agency for eligibility and details
- Don’t forget to inform your manager and Human Resources of your upcoming leave
How to pay for benefits
If you are on a LOA, the Company will direct bill you for most of your benefits. Your contributions to the HSA, Health Care and Dependent Care FSAs, and commuter benefits will not be direct billed while you are on a LOA.
When are payments due
Direct billing starts the pay period after your leave begins, or the first day of the second pay period if you’re paid weekly. You’ll receive monthly statements with biweekly or weekly coupons based on your normal pay schedule, prorated from your leave start date. You may pay each coupon individually or all at once. A 30-day grace period applies from each invoice due date.
All payments must be made in full by the end of the grace period to avoid losing coverage
Making payments
You’ll receive monthly invoices at your primary address. If you’ll be staying somewhere else during your leave, contact Billing Services to update your temporary address. You can pay by:
- Mailing a check or money order with your invoice
- Billing Services, P.O. Box 860620, Minneapolis, MN 55486-0620
- Or send an overnight delivery to Lockbox 860620, 1200 Energy Park Dr, St Paul, MN 55108
- Setting up recurring ACH payments via the LSC Benefits Center
- Log in, click Manage Billing and Payments, then Manage Accounts
- Making a one-time online payment via the LSC Benefits Center
- Log in and click Pay Now
Returning to work
Direct billing ends the day before the pay period in which you return to work. Any unpaid premiums from your leave will be collected through payroll deductions, and any prepaid premiums will be refunded by Billing Services.
When you return, your HSA, Health Care FSA, and Dependent Care FSA contributions will start being deducted from your pay check. You do not have to re-enroll to start your contributions again.
Life Insurance coverage
If you increase your Life Insurance coverage while on a leave of absence, the increase will not take effect until you’re actively back at work.
